Corrective Action Taken:

Bulbs have been replaced and u.v banks have been cleaned and disinfected. The weir after disinfection (before sample point) has been drained, cleaned and disinfected multiple times. Today I received new butterfly valves for the bank that was not in service and replaced them so that eliminates the possibility of anything leaking by when the bulbs aren't on.
