Corrective Action Taken: Bulbs have been replaced and u.v banks have been cleaned and disinfected. The weir after disinfection (before sample point) has been drained, cleaned and disinfected multiple times. Today I received new butterfly valves for the bank that was not in service and replaced them so that eliminates the possibility of anything leaking by when the bulbs aren't on.

Description Of Incident: We have contamination in our U.V system from 2 failing butterfly valves that raw effluent was leaking through on the secondary u.v bank that was not in operation.
