Description Of Incident:
|
Plant was offline. Came in and saw effluent going over weir out of the Chlorine Contact Chamber. Determined the source to be a telescoping valve that is fed from a bypass Tee valve from lagoon 3. The bypass valve had been open for over a year, but due to a high lagoon level it hadn't been noticed until now.
